Does the subject have a license for the selected profession or field of licensure?
To report an action against a practitioner who does not hold a license related to their field of practice:
- Select the type of license related to the action taken.
- Select No, for "Does the subject have a license for the selected profession or field of licensure?"
Add any other health care-related licenses the individual holds by selecting Additional License.
