What data fields are required to submit a query?
Complete all data fields for which you have the information. Complete and accurate query data allows the NPDB to process your query and return a response quickly, usually within seconds of submission. Leaving fields blank or filling in fields with incorrect information may cause the query response to be delayed (up to one business day), or the query itself to be rejected.
