What if a licensing board is asking for paper Self-Query?
The NPDB encourages all state licensing boards to accept digitally certified Self-Queries. Digitally certified Self-Queries provide assurance that your Self-Query response is unaltered and displays the exact information that is contained within the NPDB, have faster response times with no extra costs required for mail, and do not require physical processing or storage.
In the event that a state licensing board requires a paper copy of your Self-Query response, you must request a paper copy be mailed to your home or work address when paying for your Self-Query and pay the additional $13.00 fee.
