How do I get my digitally certified Self-Query response?
After your Self-Query request has been processed, sign in to your Self-Query order to download your digitally certified response.
Once you download and save the file, use Adobe Acrobat Reader to open the file and view the certification information. An error message will be displayed if any content has been altered. You must download and save the file to see the certification information . PDFs viewed in a browser will not display the certification. Printed copies of Self-Query responses will not display the certification.

Note: The NPDB will not mail or email your response to a third party, such as a state licensing board. State Licensing Boards may query the NPDB as explained in Chapter D of the NPDB Guidebook.
