Can you mail or email my Self-Query results directly to a state licensing board?
No. The NPDB will not mail or email your response to any third party, such as a state licensing board. State Licensing Boards may query the NPDB as explained in Chapter D of the NPDB Guidebook.
Note: Mailed paper copies can only be sent to a U.S. address, including U.S. territories and military addresses.
